Kathryn A. Cizek, OD
Kathryn A. Cizek, OD is licensed as an Optometrist with the State of Michigan and is a member of the Michigan and American Optometric Association. Her licensure includes providing prescriptions for Diagnostic and Therapeutic Pharmaceutical agents. She received Fellowship status in 1999 with the American Academy of Optometry.
A graduate of Indiana University, Dr. Cizek received a BS in Visual Science and achieved her Doctor of Optometry in 1986.
